ABSTRACT

Objective:To investigate the efficacy of antibiotic-loaded bone cement combined with debridement and vacuum sealing drainage in the treatment of diabetic foot ulcer wounds.Methods:Sixty-four patients with diabetic foot ulcers who received treatment in Zhejiang Rongjun Hospital from January 2018 to January 2020 were included in this study. They were randomly assigned to undergo either debridement treatment + vacuum sealing drainage (control group, n = 32) or antibiotic-loaded bone cement + debridement + vacuum sealing drainage (observation group, n = 32). The number of patients positive for bacterial culture at different time points and treatment-related indices were compared between the two groups. Results:At 3, 6 and 9 days after admission, the positive rates of bacterial culture in the observation group were 37.50%, 15.63% and 0.00%, respectively, which were significantly lower than 62.50%, 40.62% and 12.50% in the control group ( χ2 = 4.00, 4.95, 4.27, all P < 0.05). The number of operations in the observation group was significantly lower than that in the control group [(2.96 ± 0.38) vs. (3.63 ± 0.69), t = 4.81, P < 0.001). The length of hospital stay in the observation group was significantly shorter than that in the control group [(34.74 ± 3.03) days vs. (45.18 ± 2.41) days, t = 15.25, P < 0.001). The duration to wound healing in the observation group was significantly shorter than that in the control group [(58.23 ± 10.12) days vs. (72.69 ± 11.21) days, t = 6.06, P < 0.001]. Conclusion:Antibiotic-loaded bone cement combined with debridement and vacuum sealing drainage is effective in the treatment of diabetic foot ulcer. The combined treatment can effectively prevent infection of ulcer wounds, reduce the number of operations, and thereby shorten the length of hospital stay.

ABSTRACT

To investigate the clinical diagnostic value of ultrasonographic superb micro‐vascular imaging ( SM I) for abdominal branch arteries′involvement in Stanford B aortic dissection ( AD ) . Methods A total of 34 AD patients underwent conventional ultrasound ,SMI and CTA . The characteristics of the ultrasonographic images of branch artery involvement including coeliac artery ,superior mesenteric artery ,left and right renal artery were analyzed . T he diagnostic performance of conventional ultrasound and SM I were compared and CT A was used as the reference standard . Results Among the 34 patients with AD ,according to the ultrasonographic images′characteristics ,136 branch arteries were categorized into four classes :Class Ⅰ (81/136) ,branch artery perfusion supplied from the aortic true lumen ; Class Ⅱ ( 27/136) , dissection extending into the branch ; Class Ⅲ ( 16/136 ) ,branch artery perfusion supplied from the aortic false lumen ; Class Ⅳ ( 12/136) ,branch artery perfusion supplied from the aortic true and false lumens . T he diagnostic accordance rate of the conventional ultrasound and SM I were 76 .5% ( 104/136 ) and 92 .6%( 126/136) and had significant differences ( P <0 .001) . Conclusions SMI can be used to evaluate branch artery involvement in AD patients w hich will guide the practice of pre‐operation and post‐operation .

ABSTRACT

Objective To study the clinicopathological features and differential diagnosis of bizarre parosteal osteochondromatous proliferation (BPOP). Methods The clinicopathological features of 3 patients with BPOP were analyzed , and related literature was reviewed. Results 3 patients included 2 males and 1 female with an average age 40 years old (24-53 years old). The lesions in three patients were located in the left side of the distal radius, the left side of the little finger inside and the right side of ulna coracoid, respectively. Histologically, characterized by unusual mineralized cartilaginous matrix (`blue bone'), the lesion was composed of a heterogeneous mixture of bone, cartilage and fibrous tissue. Conclusion BPOP is a rare benign surface osteocartilaginous lesion, and its diagnosis and differential diagnosis mainly depend on radiographic and histopathologic features.

ABSTRACT

Aim To quantitatively analyze the lymphocyte proliferation using the time-series data of carboxyfluorescein diacetate succinimidyl ester(CFSE).Methods The model of immune suppression in mice was created by cyclophosphamide and CFSE dye was used for staining the lymphocyte of both control group and treatment group animals. The data were analyzed through mathematical model-fitting.Results The first generation of cell proliferation of the control group and treatment group were 27.17 h and 22.88 h; cell death rates in each division were respectively 20% and 40%; the half-life of cells before the proliferation was respectively 31.53 h and 43.32 h.Conclusions The mathematical fitting of CFSE data can quantitatively analyze the mechanism of the drugs attecting the proliferation of lymphocyte.In this experiment the mechanism of cyclophosphamide inhibiting lymphocyte proliferation may be due to the increase of cell death rate of each division caused by cyclophosphamide.
